If you have been off work for up to 7 days, you will require a self certificate. Your employer can provide this or click this link Self certificate
If you have been off work for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness to Work’ Certificates. The doctor is unable to forward date these certificates and so please request the fit note as close to the day it is due as possible. Please allow 2 working days for these to be processed. This will not delay your sick pay as fit notes can be backdated.
The doctor may want to speak to you or see you before issuing a fit note and the practice will contact you if this is the case.
